个人数据保护通知

此个人数据保护通知（“通知”）阐述了EMR学院（EMR）如何收集、使用和保护您提供给EMR的个人数据。本通知是根据《2010个人资料保护法》的规定编写的。

  1. 个人资料

1.1       您提供或已提供给EMR的“个人资料”包括您的姓名，联系方式，地址，电子邮件地址，身份证号码或护照号码，性别，年龄，出生日期，职位以及任何直接或间接通过您/或EMR可从通过收集、记录、保存与存储您的信息中识别出您的身份。

1.2       除了您直接提供给EMR的个人数据之外（包括在EMR举办的活动/研讨会/会议/课程/工作坊/或在向EMR提出申请/或与EMR进行任何沟通时），EMR还可能会从各种其他合法来源收集您的个人数据。

  1. 处理个人资料的目的

2.1   您的个人资料将由EMR以下述方式和目的进行使用和处理（如适用）：

  1. 出于任何，和所有向EMR提出的申请的目的;
  2. 使EMR能够执行或执行任何服务或作用，用于任何申请，查询和/或处理任何与EMR的申请;
  3. 用于注册学员/或有关的小组/或与EMR有关的任何机构;
  4. 用于EMR的内部管理目的;

2.2   您是自愿将您的个人资料提供给EMR。 但是，如果您不提供个人资料，EMR将无法：

  1. 处理您的申请/或向您向EMR要求的任何服务;
  2. 向您提供您向EMR要求的通知，邀请/或学材，无论是否与EMR的活动/研讨会/会议/课程/研讨会有关; /或
  3. 有效地与您联系。

2.3   EMR也可以处理您的个人数据，其目的如下：

  1. 向您发送更新的资料，通讯或文章; 最新推广的课程/工作坊/活动/研讨会，/或

3.3   EMR应根据需要向任何法定机构，监管机构/或其他政府机构披露和转移信息。

3.4   任何提供于EMR的个人资料，应按本通知所载的方式使用和处理。

3.5   如果您不希望通过EMR网站上的cookies收集您的个人信息，请配置您的互联网浏览器以删除或停用cookies。

4.1   您提供给EMR或EMR取得的个人资料应由EMR通过合理和适当的管理，安全措施和程序进行保护，以防止未经授权或非法处理，丢失或损坏您的个人资料。

I hereby agree and consent that EMR Academy Sdn Bhd (hereinafter known as EMR) may collect, use, disclose and process my personal information set out in my registeration form, and/or otherwise provided by me or possessed by EMR, for one or more of the purposes as stated in EMR’s Privacy Policy.

PERSONAL DATA PROTECTION NOTICE

This Personal Data Protection Notice (“Notice”) sets out how the EMR Academy Sdn Bhd (“EMR”) collects, uses and protects your Personal Data which you have provided to EMR. This Notice is prepared in accordance with the requirements of the Personal Data Protection Act, 2010.

  1. PERSONAL DATA

1.1       The “Personal Data” that you provide or have provided to EMR includes your name, contact details, address, email address, NRIC number or passport number, gender, age, date of birth, job title and any information which relates directly or indirectly to you, from which you may be identified or may be identifiable from that information, which is collected, recorded, held or stored by EMR.

1.2       In addition to the Personal Data that you provide to EMR directly (includes at events/seminars/conferences/courses/ workshops held by EMR and/or at the time of making any application to and/or any communication with EMR, as applicable), EMR may also collect your Personal Data from various other lawful sources.

  1. PURPOSE OF PROCESSING OF PERSONAL DATA

2.1       Your Personal Data will be used and processed by EMR, in the manner and for the purposes as described below (if/as applicable):

  1. for the purpose of and all purposes with regard to/in connection with any application made to EMR;
  2. to enable EMR to carry out or perform any services or functions for the purposes of and/or in connection to any application, inquiry and/or processing any application with EMR;
  3. for registration for membership and/or panel with/ or any body in relation to EMR;
  4. for EMR’s internal administrative purposes;
  5. for delivering notice(s) in relation to any changes that may affect you;
  6. for EMR to communicate with you or contact you for all purposes set out above; and
  7. for EMR to comply with any legal or regulatory requirements under any applicable laws, regulations, by-laws, guidelines, directions or any court order applicable to EMR. 

2.2       It is voluntary for you to supply your Personal Data to EMR. However, if you do not provide your Personal Data, EMR will not be able to:

  1. process your application and/or provide you with any services that you requested from EMR;
  2. provide you with the notices, invites and/or materials that you requested from EMR, whether or not in relation to EMR’s events/seminars/conferences/courses/ workshops; and/or
  3. communicate effectively with you.

2.3       EMR may also process your personal data for the purposes as follows:

  1. to send you materials such as updates, newsletters or articles; latest promotion of courses/ workshops/events/seminars, and/or
  2. for delivery to you invites to EMR and/or its worldwide affiliates’ related events, materials or any specific information in connection thereto; if any, by way of email, fax, telephone, sms and/or postal mail.

2.4       If you do not agree to EMR in processing your Personal Data for any of the purposes set out in the above paragraphs, please notify EMR by contacting EMR.       

  1.         DISCLOSURE OF PERSONAL DATA

3.1       Your Personal Data provided to EMR or obtained by EMR shall be kept confidential by EMR.

3.2       EMR may disclose and/or publish your Personal Data if you have attained membership and/or panelship with EMR in EMR newsletters, website, social media and/or any other related publications of EMR, if applicable.

3.3       EMR shall also disclosed and transfer the information to any statutory bodies, regulatory bodies, and/or other governmental authorities where required.。

3.4       Any Personal Data sent to EMR shall be used and processed in the manner set out in this Notice.

3.5       If you do not wish for your Personal Data to be collected via cookies on EMR’s website, please configure your internet browser to delete or deactivate cookies.      

  1. SECURITY AND RETENTION OF PERSONAL DATA

4.1       Your Personal Data provided to EMR or obtained by EMR shall be kept secured by EMR through reasonable and appropriate administrative and security measures and procedures to prevent any unauthorized or unlawful processing, loss of or damage to, your Personal Data.

  1.         REQUEST FOR ACCESS AND INQUIRIES

5.1       EMR takes reasonable steps to ensure that your Personal Data provided to EMR is accurate. However, you may at any time hereafter request for access to, or request for rectification or correction of your Personal Data, or limit the processing of your Personal Data by EMR, as the case may be, however subject to the exceptions and restrictions as may be contained under the applicable law. 5.2       Please note that EMR has the right to refuse your request to access and/or make any correction to your Personal Data to the extent permitted under the applicable law.         

  1. AMENDMENT TO NOTICE

EMR reserves the right to amend or modify this Notice from time to time without prior notice to you.
